About the Role
Central Cheshire Integrated Care Partnership (CCICP) is expanding its Advanced Community Practitioner team to support the Urgent Community Response and Virtual Ward services on a permanent basis.
Responsibilities- Conduct home visits to patients who are acutely unwell and at risk of hospital admission.
- Assess, diagnose and treat patients autonomously as part of a multidisciplinary community team.
- Collaborate with District Nurses, Macmillan Nurses, არსებించుబుదువ Industry Complex Case Practitioners, physiotherapists, occupational therapists and other members of the Advanced Practice team.
- Participate in quality and safety improvement projects to advance patient safety and service quality.
- Valid HCPC or NMC registration.
- Full UK driving licence and access to a vehicle suitable for community work.
- Willingness to work shift patterns covering 08:00‑20:00, seven days a week, including bank holidays.
Applicants who do not meet the personal specification but possess relevant clinical experience may be considered for a developmental/training role.
