Occupational Therapist - Rehab

We look forward to welcoming a band 5 Occupational Therapist to join our rehabilitation team.

You will be supported by the wider rehabilitation team, with access to opportunities to develop in a variety of settings, be part of a robust training and preceptorship programme, and regular supervision.

Main duties of the job

To undertake assessment of patients with a wide range of conditions and to work as autonomous clinicians, under the guidance of Clinical Specialist Occupational Therapists as required, using accurate analytical skills and clinical reasoning to interpret assessment findings, identifying patient problems, setting goals, and providing an appropriate treatment plan and progression.

To work collaboratively with the patient, members of the multidisciplinary team and multiagency team to achieve the best possible outcome for the patient.

Mid Cheshire Hospitals NHS Foundation Trust (MCHFT) provides good quality, safe and effective healthcare to the people of Cheshire and beyond. The Trust, which manages Leighton Hospital in Crewe, Victoria Infirmary in Northwich, and Elmhurst Intermediate Care Centre in Winsford, was established as an NHS Trust in April 1991 and became a Foundation Trust in April 2008. Job responsibilities

To work autonomously to manage a defined patient caseload whilst working as part of a multidisciplinary team, referring to a senior clinician who is available on a daily basis as required.

To contribute to the delivery of the service in a variety of defined clinical settings, which may include in-patient wards, out-patient and community clinics and the patients home.

To utilise effective communication skills to ensure clinical advice and treatment programmes are delivered sensitively, taking into consideration the needs of carers.

To maintain professional practice within the context of the Health & Care Professions Council. This includes standards of proficiency, conduct and performance, ethics and continuing professional development.

To work as a member of the multidisciplinary team, contributing to decisions with regard to patient care programmes through attendance at ward rounds, case conferences and other multidisciplinary meetings.

To maintain accurate clinical documentation in line with professional standards, and local policies and procedures.

To provide accurate and timely information and reports to referring agents and other agencies where appropriate.

To maintain competency to practice through CPD activities, producing a personal portfolio which reflects professional development.

To record and submit clinical activity data as required by the Trust in an accurate and timely manner.

To contribute innovate ideas to enable the development and implementation of service improvements.

To contribute to the supervision, education and assessment of the performance of students, providing timely verbal and written feedback as required.
