Diabetes Inpatient Specialist Nurse

As a specialist nurse you will participate in the strategic planning, development and delivery of a Diabetes and Endocrinology service, working in partnership with other specialist nurses in both acute and primary care.
The post holder will devise, initiate and further develop educational programmes for all grades of staff in all areas of diabetes management.
As a registered nurse you will be expected to consistently meet the performance standards as set out in the Code of Professional Conduct (NMC 2015).
Main duties of the jobProvide advice, information, education and support to patients and/or relatives of people with diabetes.
Ensure effective communication throughout the trust to develop and build working relationships with other health care professionals to facilitate best practice in the management of patients with diabetes.
Keep clear and concise records of all actions taken and support given within your role as a specialist nurse.
Ensure attendance and participation in the specialist nurse group meetings.
Ensure confidentiality is maintained without compromising patient care.
Effectively manage the day‑to‑day provision of the diabetes service at Mid Cheshire Hospitals NHS Trust hospitals observing budgetary limitations and standing financial instructions.
Ensure appropriate forecasting to take account of future service development.
Network internally and externally with other professionals and specialist nurses to identify best practice.
Collaborate with the Lead Nurse and members of the Nursing Strategy Groups during regular reviews of nursing standards and trust policies.
Collaborate with the Lead Consultant, the Divisional Lead Nurse and other members of the diabetes service during the development of diabetes guidelines and policies.
Job responsibilities SERVICE DELIVERYProvide advice, information, education and support to patients and/or relatives of people with diabetes in line with Best Practice Tariff, including 7‑day service provision.

GOVERNANCEUtilise an evidence‑based approach to the management of patients’ health and illness, including the application of up‑to‑date research findings relevant to the management of the client group.
Participate in research mapped to service need, benchmarking and equipment trials (as appropriate) to improve effectiveness of patient care.
Disseminate research and audit findings through presentation to professional groups.
Contribute to the development and implementation of policies, procedures and guidelines relevant to own area of work.
Participate in the clinical supervision of colleagues on an individual or group basis.
Promote the involvement of patients and their families with health‑related activities that contribute to the improvement of services.
Promote and evaluate practice to ensure patients’ equality, diversity and rights are maintained and that practice is professional, ethical and equitable.
