Certified Nurse Aide – Quality Private Duty Care
The Certified Nurse Aide (personal care attendant) is a trained employee who provides personal care and related services in the home. He or she functions under the direction, instruction, and supervision of the Director or Assistant Director of Patient Services and/or the RN Team Leader.
Qualifications- Completion of a basic aide training program consistent with state requirements and possession of a certificateল্পে.
- Working knowledge of the home health care program and prior experience providing home health visits preferred.
- Ability to read and write consistent with job requirements.
- Completion of PCA training covering the following areas:
- Methodology of assisting clients to achieve maximum self-reliance.
- Principles of nutrition and meal preparation.
- Principles of the aging process.
- Understanding the emotional problems of illness and long‑term home care.
- Procedure for maintaining a clean, healthful and pleasant environment.
- Reporting changes in client condition to the nurse.
- Maintaining the ethics and confidentiality of client care.
- Cooperation with the health care team.
- Providing and maintaining appropriate records.
- Comprehending the basics of personal care, housekeeping and meal preparation.
- Showing an interest and concern for people.
- Practicing good hygiene and maintaining a neat appearance.
- Freedom from health problems that may be injurious to the client, self or co‑workers; presentation of required health documentation.
- Appropriate work experience and emotionalinement required for establishing and maintaining good work relationships with clients, clients’ families, and agency personnel.
- Reliability of transportation, valid TN driver’s license, auto insurance as required by the agency, and a safe driving record. pwm?
- Helps the client maintain good personal hygiene.
- Plans and prepares nutritious meals, provides for client shopping needs, and completes other errands as instructed by the supervisor.
- Assists client with ambulation, certain treatments, and rehabilitative processes as ordered by the physician and approved by the nurse.
- Encourages the client to become as independent as possible according to the nursing care plan.
- Promotes client’s mental alertness through involvement in activities of interest.
- Provides simple emotional and psychological support to the client and other household members.
- Establishes a trusting relationship with the client and family.
- Prepares a daily visit report for inclusion in the clinical record.
- Reports changes in the client’s mental or physical condition to the immediate supervisor.
- Reports inability to carry out assigned tasks to the supervisor.
- Works with personnel from other agencies as directed.
- Performs routine housekeeping tasks related to a safe and comfortable environment.
- Follows universal precautions when providing care.
- Maintains confidentiality of business practices, activities, and personnel information.
- Performs only those functions specified for each client.
- Responds to the client’s physical, emotional, and developmental needs.
- Follows emergency procedures in the event of an incident, injury, or significant change in the client’s condition.
- Attends team conferences when directed and participates in patient care discussions.
- Ensures adherence to all organizational policies, procedures, and processes and seeks clarification when needed.
- Provides input on the evaluation of other team members’ performance when requested.
- Participates willingly in special projects and overtime as requested.
- Maintains a professional attitude and appearance when working with staff.
- Provides proper notification for absences or tardiness and prevents personal affairs from interfering with work.
