Sussex Community NHS Foundation Trust is seeking a Frailty Nurse to join the Adult Community Nursing team for a maternity cover. The post involves autonomous practice, using expert frailty knowledge to assess, plan, implement, and evaluate nursing care for patients in the community.
You will work with patients, families, GPs, and multidisciplinary teams to prevent hospital admissions, manage complex needs, and provide leadership and education within the team.
