We are seeking a Physical Therapist to join our team at Acadia Extended Care Hospital in Crowley, LA. This is a direct hire, full‑time opportunity focused on high‑acuity patients recovering from complex medical conditions in a Long‑Term Acute Care setting.

Physical Therapist – Long‑Term Acute Care (LTAC, ICU, CC)

• Full‑time position – Day shift

• Deliver evidence‑based, hands‑on physical therapy to medically complex patients recovering from serious illness, injury, or surgery
• Collaborate closely with physicians, nurses, respiratory therapists, and the interdisciplinary care team to implement and adjust individualized rehabilitation plans
• Assess patient mobility, strength, endurance, and functional capacity, and develop targeted treatment strategies to maximize recovery and independence
• Educate patients and caregivers on mobility techniques, fall prevention, and safe discharge planning to promote long‑term outcomes
• Accurately document patient progress and treatment effectiveness in compliance with facility policies, regulatory standards, and professional best practices

• Active Physical Therapist license in Louisiana
• 1+ year of acute care, ICU, or LTAC experience strongly preferred
• BLS required
• Strong clinical judgment and a commitment to excellence in patient care
