The LPN (or RN) reports directly to the Director of Nursing and is responsible for delivering, coordinating, and evaluating resident care. The role combines hands‑on clinical duties—medication administration, resident assessments, documentation, and direct support—with leadership responsibilities such as guiding care staff, promoting safety initiatives, and ensuring effective communication with residents, families, physicians, and community leadership.
Essential Functions- Provide services in accordance with the nurse practice act and regulatory standards.
- Collaborate with the Director of Nursing to direct and supervise care staff.
- Review and revise assessments and care plans at admission and periodically.
- Interpret physician orders and administer medical treatments as directed.
- Initiate resident education plans per physician and community policy.
- Monitor resident condition changes and communicate promptly to physicians and supervisors.
- Administer medications following the five rights and supervise medication administration for self‑administering residents.
- Document all observations, interventions, and clinical procedures throughout the shift.
- Assist residents with nutrition, hydration, activities of daily living, and scheduled medical appointments.
- Use proactive interventions, monitoring, and safety practices during rounds.
- Respond to emergency situations per nursing standards and protocols.
- Maintain a restraint‑free environment and support fall prevention programs.
- Assist with transfers and ambulation using proper body mechanics.
- Collect vital signs and assess residents’ conditions as requested.
- Support medication delivery process, including order entry, receipt, and daily MAR documentation.
- Notify pharmacy and supervisors of medication issues and seek timely resolution.
- Promote residents’ physical and emotional well‑being and strive for maximum independence when possible.
- Follow residents’ advance directives and ensure effective family communication.
- Engage in conflict resolution and collaboration with residents, families, and staff.
- Perform additional duties as assigned.
- Graduation from an accredited nursing school and a current state license (LPN or RN).
- 1+ year of nursing experience preferred; new graduates welcome.
- CPR or BLS certification preferred.
- Proficiency with common digital tools:
Microsoft Office, web‑based platforms, electronic health record systems, and HR systems.
- Work may involve walking or standing for extended periods and moving throughout community areas.
- Tasks include stooping, kneeling, crouching, crawling, climbing, or balancing at times.
- Effective communication and reading skills are required.
- Ability to lift up to 50 pounds independently, and up to 200 pounds with assistance.
- Strict adherence to infection prevention protocols and PPE usage.
- Active participation in staff teamwork and morale activities.
- Ongoing education and training are required, with a commitment to continuous learning.
