Support Worker / Outreach Worker
An exciting opportunity to join a dedicated supported housing and outreach service, providing person‑centred support to individuals with complex needs, including homelessness, mental health challenges, and substance misuse issues.

Salary: £13.00 to £14.50 per Hour
Location:
Work across various sites in Lewisham, Croydon, Lambeth, and Southwark
Job Type: Full-Time, Permanent
Duties- Deliver tailored support:
Provide one‑to‑one support for individuals with complex needs, promoting independence and personal development. - Encourage independence:
Support clients to build essential life skills such as budgeting, maintaining a tenancy, and daily living tasks. - Promote wellbeing:
Monitor and respond to residents’ health and emotional wellbeing needs, ensuring access to relevant services. - Safeguard vulnerable people:
Identify and report any safeguarding concerns promptly in line with policy and procedure. - Develop support plans:
Co‑create person‑centred support and risk‑management plans with residents using SMART objectives. - Collaborate with agencies:
Work with housing, healthcare, probation and social care professionals to deliver wraparound support. - Champion inclusion:
Promote equality, diversity, and social inclusion, challenging stigma and discrimination. - Ensure a safe environment:
Carry out wellbeing and security checks within supported accommodation and during outreach visits. - Maintain accurate records:
Keep thorough written and electronic documentation in line with organisational standards and GDPR.
- Proven experience supporting people with complex needs, including homelessness, mental health, or substance misuse.
- Understanding of supported housing and outreach‑based support models.
- Strong communication and report‑writing skills.
- Ability to work flexibly, independently and as part of a wider team.
- IT proficiency for maintaining accurate case notes and reports.
- Qualification in Health and Social Care Level 2 or above.
- Experience working in supported housing or residential care.
- Understanding of welfare benefits and housing processes.
- Knowledge of the Care Act and Mental Health legislation.
- Ability to cook and support residents in meal preparation.
This role requires a DBS check.
