Enhance Rehabilitation is currently seeking qualified Physical Therapist candidates to join our dynamic team. As an Enhance Rehabilitation Physical Therapist, you will be part of a growing team providing the highest quality care for patients in SNF settings. In this role, you will evaluate and treat patients following an individualized plan of care.
Responsibilities:- Provide excellent care for our patients
- Deliver excellent customer service to patients and family members
- Collaborate with facility staff to ensure optimal patient care
- Evaluate new patients, develop plans of care, and set measurable, functional goals
- Treat patients, address their concerns, and educate them on their injury, progress, and probable outcomes
- Supervise Physical Therapist Assistants
- Re-evaluate patient progress, revise plans of care, and reset goals
- Complete timely and accurate documentation including evaluations, re-evaluations, discharges, and daily progress notes
