We are seeking a Home Health RN Case Manager to provide care in West Allis, Oak Creek, and Cudahy.

• Make the initial nursing evaluation and create an appropriate, patient-specific plan of care.
• Identify the patient’s/family's physical, psychosocial, and environmental needs and tailor each care plan appropriately.
• Complete recertifications or resume care for eligible patients.
• Initiate and coordinate the plan of care while maintaining accurate and relevant clinical notes regarding the patient's condition.
• Document problems, appropriate goals, interventions, and patient/family response to skilled nursing care.
• Collaborate with the patient/family, attending physician, and other members of the IDG in providing patient and family care.

• One year of clinical experience preferred.

• Current Wisconsin RN licensure.
• Current driver’s license, valid vehicle insurance, and access to a dependable vehicle.

• Schedule:
  
  Full-Time, 1st shift Monday-Friday. Minimal on-call required.
• Compensation:
  Negotiable based on experience.
• Sign-On Bonus:
  Up to $7,500 available, depending on location and experience.
