- Provide care to patients according to plan under supervision of RN.
- Assess patients' condition making necessary adjustments in patients' plan of care related to symptom changes and changes in care needs.
- Act as liaison for patients/caregivers with MD and community healthcare partners.
- Complete all documentation related to patients' contact, assessment and care.
- Maintain understanding of current home care regulatory guidelines.
- Make necessary hospice visits as assigned.
- Demonstrate and encourage team behavior and exceptional patient/guest experiences.
- Uphold and promote patient safety and quality.
Job Summary

Graduate from an accredited school of practical nursing and currently licensed in the state of Alabama required.
Experience
Minimal 2 years' experience in a hospital setting working in Med Surg, critical care or ER. Previous home care or hospice experience preferred.
Additional Skills/Abilities
Must exhibit strong critical thinking and organizational skills. Must be able to work independently and make independent decisions. Must be flexible in scheduling. Must be able to organize and prioritize schedule to meet patient needs.
