Are you a new or seasoned LPN with a passion for mental health looking for a full-time role? At Easterseals PORT Health (ESPH), our mission is rooted in empowering individuals and strengthening communities. We are seeking compassionate and motivated full-time Licensed Practical Nurses to join our Virginia REACH team in supporting youth with I/DD and mental health needs at our warm, home-like Youth Crisis Therapeutic Home (YCTH) in Culpeper, VA.

• Monday-Friday 3pm-11pm
• Monday-Friday 11pm-7am

• Provide compassionate medical and nursing support to youth with intellectual/developmental disabilities and co-occurring behavioral health needs
• Administer medications safely and accurately in accordance with clinical protocols
• Conduct nursing assessments
• Educate and empower individuals regarding their mental and physical health
• Collaborate closely with interdisciplinary team members to ensure coordinated, person-centered care
• Maintain thorough and timely documentation of all nursing services and observations
• Uphold compliance with all regulatory and clinical care standards to ensure the highest quality of service

• Competitive rate: $28.00 - $36.00 per hour for this full time, non-exempt position

• Licensure:
  Current valid, unrestricted licensed as a Practical Nurse in state of Virginia
• Knowledge: MH and I/DD systems of care preferred
• Skills:
  Strong communication and documentation
• Experience:
  
  Crisis prevention and intervention services preferred
• Transportation: A valid driver's license and car insurance for occasional transportation to appointments
