Case Manager RN

Job Summary

The RN Case Manager serves as a leader of the complex discharge planning process and a valued member of the healthcare team. They collaborate with physicians and other members of the healthcare team to assess, plan, implement and evaluate proactive/complex/ safe discharge plans. The RN Case Manager is also responsible for facilitating coordination of care and serves as a resource for discharge planning and care coordination for medically complex cases.

• Synthesizes data to determine preliminary diagnosis and therapeutic plan utilizing principles of prevention.
• Recommends community resources to meet patient and family needs.

• Monitors quality of core processes in department.
• Applies the principles of safety and infection control to ensure the safety of all patients.

• Works within a team to achieve patient and team goals.
• Shares and initiates regular and professional communication with co-workers.
• Participates in regular staff meetings.
• Identifies opportunities to improve processes based on feedback from staff and customers.

• Obtains complete history and physical data on patients.
• Reviews patient records.
• Interprets and integrates data to determine appropriate diagnostic and therapeutic procedures needed.
• Performs a complete physical exam and records findings.
• Establishes presumptive diagnosis.
• Orders appropriate laboratory and diagnostic procedures.
• Performs independent evaluations and treatment procedures in emergency situations.
• Provides monitoring and continuity of care between physician visits.

• Associate's Degree in Nursing required. 4 Year/Bachelor’s Degree, BSN, preferred.
• 2+ years of healthcare experience preferred. Case Management or related experience preferred.
• Current RN licensure in state of employment required.
• Weekend and holiday coverage.
• Knowledge/Competence of Microsoft Office products and keyboarding.

• Medium to Heavy Physical Demand.
• Constant standing, frequently walking, bending/stooping.
• Ability to lift/push/pull 100 lbs. For weight over 35 lbs., use Safe Handling Equipment.
• Use of safety equipment such as HEPA mask for infection prevention:

• On call responsibilities:

• Travel between campus buildings, remote facilities, and out of town as needed:
  Yes.
