Home Health Links is seeking a Physical Therapist Assistant to treat patients in the Home Health setting. Our company is therapist-owned and operated, so we understand what it takes to be in the field.

What you do:

• Follow the plan of care provided by the Physical Therapist and deliver treatment to patients.
• Interpret evaluation findings to provide care and ensure safety relating to dysfunction.
• Provide treatment based on established goals, ensuring follow-through to achieve these goals.
• Advise and educate patients and their families.
• Regularly assess clinical signs and symptoms to determine treatment effectiveness and patient progress towards goals, modifying plans as necessary.
• Provide discharge planning for patients from physical therapy and communicate when the patient has received maximum benefit from occupational therapy.
• Document physical therapy patient management accurately and in a timely manner, demonstrating progress towards meeting professional practice criteria.

We set standard base pay ranges for all roles based on function and level. The final offer is determined by multiple factors, including skillset, work experience, and languages, and may vary from the amounts listed below.

The pay range for this position is: $60-$75/visit

• Graduate of an accredited Physical Therapist Assistant Program.
• Home Health experience preferred; not required.
