• Medical Degree (MD/DO)
• Accredited Family Medicine Residency Program Completion
• Board-certified in Family Medicine (or board-eligible for residency new graduates)
• Unrestricted Pennsylvania Medical License (or ability to obtain)
• DEA Certification
