Integrated Care Clinician (Nurse)
The post holder will undertake telephone triage assessments of patients in an efficient way. They will advise the most appropriate intervention to meet the needs of the patient using best clinical practice, adhering to Trust Policy, protocols and guidelines. The role will be performed in an autonomous capacity, assessing and advising callers within agreed guidelines.

NHS 111 Wales is the service that gives the people of Wales access to urgent out‑of‑hours care, 24‑hour health information with sign‑posting and access to urgent dental services.

This dynamic role will draw upon your clinical assessment skills to help and advise a broad spectrum of patients, from minor ailments and injuries to complex and urgent conditions, and hone your skills in a remote clinical decision‑making environment. As a Band 6 Triage and Assessment nurse, you’ll have the autonomy to make decisions while being supported by our experienced team of senior clinicians, practice coaches and multidisciplinary colleagues.
