Hospice - PRN - Registered Nurse; RN

As a Hospice Nurse (RN), you will travel to patients' homes each day to administer healthcare services, helping patients maintain independence. Duties include administering IVs, changing dressings, cleaning wounds, and performing Foley catheter insertions.

• Case management and supervised visits.
• Perform skilled and comprehensive assessment during initial admission.
• Assess and chart observations of the patient’s condition at each visit (vital signs).
• Provide skilled nursing care as needed in accordance with the plan of care (administer medication, clean/dress wounds, assess healing progress).
• Provide proper patient education for home health care procedures and strategies.
• Maintain clinical records: visit notes (within 24 hrs), OASIS assessments (within 48 hrs), medication profiles, orders/plan consistent with timely submission.
• Maintain confidentiality of patient PHI in compliance with HIPAA.
• Provide palliative comfort care.

• Current Texas Registered Nurse license.
• Minimum 2 years experience as RN in Home Health.
• Current CPR certification.
• Reliable transportation.
• Valid Texas Driver’s License and Auto Liability Insurance.
• Current TB Skin Test.
• EMR experience; preference for Kinsser.
