Talentenregio in Dalfsen is op zoek naar een wijkconsulent Zorg & Ondersteuning voor de flexibele schil. Je helpt inwoners met hulpvragen en versterkt hun netwerk, waarbij je zinvolle oplossingen biedt.
Als onderdeel van verschillende wijkteams werk je aan het vergroten van de zelfredzaamheid van inwoners en voer je integraal onderzoek uit. De functie vereist ten minste 2 jaar ervaring met de WMO en 5 jaar in het sociale domein.
Wij bieden een dienstverband van 36 uur per week met een salaris tussen de € 3.206 en € 5.033, afhankelijk van ervaring en opleiding.
