Health & Wellbeing Practitioner

Location

Midlothian GP Practices, primarily based in one or two GP practices with travel to other practices and community venues across Midlothian.

Permanent, full time

£30,356 per annum

• Work with a team of practitioners to deliver the Midlothian GP contract.
• Travel across Midlothian to engage with people referred to the Wellbeing Service.
• Maintain persistent engagement to support people who struggle to receive support.
• Manage a caseload of referred individuals.
• Apply a strengths‑based personal outcomes approach to help clients identify and access appropriate self‑management support.
• Provide specific coaching in self‑management.
• Deliver group‑based Lifestyle Management courses.
• Link individuals to community resources and supports.
• Support volunteers with lived experience of long‑term conditions into peer roles.

Health and social care practitioner with experience supporting people living with long‑term physical and mental health conditions to self‑manage.

• Capable of supporting vulnerable people experiencing significant psychosocial barriers and health inequalities.
• Strong solution‑focused coaching and group facilitation skills.
• Ability to work autonomously in community settings.
• Respects client strengths and resilience, recognising them as experts in their own lives.
