Position Description Position SummaryThe Dietician/Dietitian will provide nutritional assessments, dietary counseling, education and interventions to the terminally ill patient and their family and/or care givers. The Dietician/Dietitian is an active participant in the Hospice's Interdisciplinary Group/Team (IDG/T) and assists in developing, coordinating, and implementing an individualized Plan of Care (POC).
Responsibilities- Models a service attitude towards others, takes ownership in solving problems and takes the initiative to make things better, is friendly and courteous to residents, patients and co-workers
- Develops and implements a dietary plan of care based on the assessment of the hospice patient's nutritional needs
- Provides education and counseling for both (1) the purpose of training the patient, family or care givers in providing patient care and, (2) helping the patient and those providing care to adjust to patients’ approaching death.
- Appropriately reports changes to the IDG/T. Documents appropriate, pertinent information and interventions provided by completing notes and updating the POC to ensure continuity of care.
- Accepts clinical assignments that are consistent with education and competence to meet the needs of the patients.
- Functions within state license and standards of practice.
- Communicates and collaborates in a positive, supportive and respectful manner to patients, families, care givers and the IDG/T.
- Provides education to the patients and caregivers regarding nutrition in terminal illness and at the end of life.
- Provides an environment which promotes respect for patients, their privacy, and property.
- Participates in the Hospice’s QAPI program as appropriate.
- Demonstrates commitment, professional growth, and competency.
- Follows the Hospice’s infection control policy.
- Promotes the Hospice’s philosophy and administrative policies to ensure quality of care.
- Participates in the Hospice’s educational in-service programs as required.
- Other duties as assigned
- Bachelor’s Degree in nutrition or a related subject, preferred.
- Registered or Licensed Dietician.
- Must have current driver’s license.
- One (1) year of experience as a dietician/dietitian. Experience in hospice or working with terminally ill patients preferred.
- Knowledge of nutritional needs of the terminally ill patient. Good interpersonal skills. Demonstrates ability to work with electronic medical records. Good understanding of hospice philosophy.
- Reliable transportation and valid auto liability insurance required.

- Demonstrated proficiency in a variety of computer software applications and Microsoft Office

- Ability to communicate with all levels of teammates and provide exceptional customer service.
- Ability to maintain strict confidentiality; able to handle confidential information and use good judgement when making decisions
- Ability to perform work tasks within the Physical Demand Requirements
- Ability to comply with the Resident Bill of Rights and the Employee Responsibilities.
- Ability to comply with organizational and departmental safety policies and procedures.
