Program Coordinator - Shared Care Department Essential Duties and Responsibilities

  • Responsible for tracking, coordination and communication of patient referrals
  • Ensure complete and accurate registration, including patient demographics and current insurance information is accompanied by the referral
  • Ensures referrals are addressed within a timely manner
  • Schedule new patients for appointments
  • Communicate with other physician offices to ensure referrals requirements are met
  • Compile medical records, as needed
  • Answers phones to address questions from patients and staff
  • Management of compliant program documentation
  • Assist external Shared Care field Physician Liaisons as needed
  • Maintains strict confidentiality; adheres to HIPAA guidelines and regulations
  • Performs other work‑related duties as assigned
Minimum Qualifications (Knowledge,

Skills and Abilities

)
  • High School Diploma
  • 2 years’ experience with referral coordination and insurance verification
  • Knowledge of business office procedures
  • Ability to foster positive relationships with Shared Care Team and referring partners
  • Ability to operate a computer, computer software and basic office equipment
