Program Coordinator - Shared Care Department Essential Duties and Responsibilities
- Responsible for tracking, coordination and communication of patient referrals
- Ensure complete and accurate registration, including patient demographics and current insurance information is accompanied by the referral
- Ensures referrals are addressed within a timely manner
- Schedule new patients for appointments
- Communicate with other physician offices to ensure referrals requirements are met
- Compile medical records, as needed
- Answers phones to address questions from patients and staff
- Management of compliant program documentation
- Assist external Shared Care field Physician Liaisons as needed
- Maintains strict confidentiality; adheres to HIPAA guidelines and regulations
- Performs other work‑related duties as assigned
Skills and Abilities

- High School Diploma
- 2 years’ experience with referral coordination and insurance verification
- Knowledge of business office procedures
- Ability to foster positive relationships with Shared Care Team and referring partners
- Ability to operate a computer, computer software and basic office equipment
