Our experienced clinicians conduct In-Home Health Evaluations to help bridge gaps in care and help address social determinants of health for health plan members. Colleagues in our member engagement coordinator roles engage with health plan members, helping to schedule their In-Home Health Evaluations and answer questions. We offer extensive training, coaching and regular feedback to prepare every team member for success.

• Conduct In-Home Health Evaluations to help bridge gaps in care and address social determinants of health for health plan members.
• Engage with health plan members to schedule In-Home Health Evaluations and answer questions (as part of member engagement coordinator roles).
• Support the delivery of comprehensive and personalized health evaluations, including preventive screenings, to health plan members’ front doors.
