- Social Services, Provides case management services, information on public benefits, and support to the needs of the residents of Dallas County age 60+ and their caregivers. Assess client needs and develop individualized care plans that support health, safety, and independence. Requires the ability to operate at an independent level within the policies of the Community Council of Greater Dallas (CCGD), the Dallas Area Agency on Aging (DAAA) and in accordance with the laws and regulations of the Texas Health and Human Services Commission (THHSC).

General Responsibilities- Exercise discretion and independent judgement to conduct in-person needs assessments for caregivers and older adults living by themselves.
- Secure all required documentation according to DAAA Operating Procedures, to determine client needs; assist them and/or their families in accessing available resources
- Write client narrative in client tracking system and prepare client care plan. Share care plan with caregiver and coordinate services based on the care plan.
- Maintain client confidentiality. This includes client records, client data and any other document that has client PI information.
- Authorize services from vendors for client upon completion of assessment and securing required documentation.
- Submit requisitions for payment of invoices and/or vouchers in timely manner
- Submit other required reports and forms as directed
- Enter all client data for Benefits Counseling once a week if required.
- Secure certification as a Benefits Counselor I within six months of hire date
- Secure Benefits Counselor II certification as directed. Always maintain valid certification.
- Provide Benefits Counseling for clients as needed and when requested. Complete all required documentation and maintain client confidentiality.
- Assist with Benefits Counseling activities as requested by client and as directed by Program Director, Access & Assistance (i.e. Medicare Part D enrollment, presentations, etc.)
- Assist with the intake calls to DAAA. Log the calls in a "call in log" and complete a referral form for the request for service if needed or requested.
- Provide presentations to individuals and/or their representatives and groups as requested.
- Maintain current list of available resources and share information on new resources with fellow Access and Assistance staff.
- Coordinate with other Access and Assistance staff to ensure cooperation in the intake, screening and referral process.
- Work with agency affiliates or partners to ensure access to services for the caregivers.
- Represent DAAA and/or CCGD at public events as directed.
- Attend staff meetings, trainings, seminars, and workshops for professional development
- Perform other duties as assigned.
- Written & Oral Communications, fluency in English both written and spoken; fluency in a second language (Spanish, Mandarin Chinese, Vietnamese, Korean, Farsi) a plus but not required
- Basic knowledge of computers for use for word processing, formatting documents, spreadsheets, and data entry
- Demonstrated expertise of public benefits and specific eligibility criteria
- Knowledge of services and programs serving older adults and/or adults with disabilities and experience working with those populations
- Case management experience
- Ability to interact with clients from diverse populations and backgrounds
- Demonstrated tact, good judgement and ethics
This position has no direct supervisory responsibility.
Work EnvironmentThis job operates in a professional office setting. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.
Physical DemandsThis is largely a sedentary role; however, some mobility is required. This would require the ability to lift files, open filing cabinets and bend or stand as necessary and to transport materials to and from meetings, presentations and to home visits as needed. Good vision, and good hearing acuity (with corrective lenses and/or hearing aids if necessary).
