Population Nurse Manager

Position Overview The Population Nurse Manager will lead and develop a remote team of nursing leaders and clinical professionals in a fast‐paced, patient‑centered environment. This role oversees multiple Nurse Team Leads, ensuring high‑quality, efficient, and compassionate care delivery. This position operates on Eastern Time, Monday‑Friday, 8:30 AM‑5:00 PM, and requires recent experience in population healthcare and remote leadership.
Responsibilities Of The Population Nurse Manager- Manage, mentor, and coach Nurse Team Leads, providing structured performance management and leadership development.
- Support Team Leads in improving productivity, engagement, and efficiency through goal‑setting, feedback, and accountability.
- Establish, monitor, and analyze performance metrics across nursing teams to identify trends and drive improvement initiatives.
- Serve as an escalation point for complex clinical or member situations, reinforcing best practices and patient‑centered care.
- Build strong communication channels between leadership and frontline teams, incorporating staff feedback into operational decisions.
- Champion consistent, compliant workflows and documentation to maintain high standards of quality and operational excellence.
- Anticipate staffing needs, support succession planning, and develop growth pathways for nursing leaders and teams.
- Represent nursing leadership in organizational forums and advocate for strategies that enhance care quality and workforce performance.
- 8+ years of nursing experience.
- 2+ years in a leadership role within a remote case management or complex care environment.
- Recent experience managing Nurse Team Leads in a remote setting.
- Proven ability to lead teams, develop talent, and hold staff accountable to performance and quality standards.
- Experience aligning team goals with population health strategies, including risk stratification, care gap closure, and quality improvement.
- Strong capability using performance metrics and data to drive continuous improvement.
- Experience navigating organizational change and guiding teams through evolving priorities.
- Comfort with virtual communication tools, electronic health records, and productivity platforms.
- Self‑starter with strong accountability, follow‑through, and results‑driven mindset.
- Oncology or complex‑care population health leadership experience.
- Bachelor of Science in Nursing (BSN) with an active, unrestricted RN license.
- Willingness to obtain additional state licenses as required.
- Professional nursing or case management certification required (e.g., OCN, AOCN, AOCNS, CCM, or equivalent).
