Job Summary and Qualifications


* This is a Full Time Day shift position, Monday - Friday, hours are 8:00am - 4:30pm *

The Heart Failure Coordinator is a registered nurse responsible for the comprehensive coordination of care for patients with heart failure across the continuum of care, from inpatient admission through outpatient follow-up. This role ensures the delivery of evidence-based, patient centered care that aligns with accreditation standards. The coordinator collaborates with multidisciplinary teams to optimize patient outcomes, reduce readmissions, and promote adherence to clinical guidelines.

Key focus areas include care coordination, patient and staff education, performance measure monitoring, and quality improvement initiatives to support disease-specific certification.

What You Will Do In This Role:
  • Oversee the heart failure program, ensuring compliance with national standards and guidelines.
  • Coordinate data collection, analysis, and reporting related to heart failure patients.
  • Serve as a liaison between medical staff, nursing staff, and administration regarding heart failure protocols.
  • Develop and implement educational programs for staff and patients related to heart failure management.
  • Monitor and evaluate the effectiveness of heart failure protocols and procedures.
  • Collect and analyze data related to heart failure patient outcomes.
  • Prepare reports and presentations on the heart failure program performance.
  • Conduct regular interdisciplinary meetings.
  • Collaborate with physicians and other healthcare professionals to improve patient care.
  • Develop and implement educational programs for staff on heart failure management.
  • Provide education and support to patients and families regarding heart failure.
  • Participate in quality improvement initiatives related to heart failure care.
  • Maintain current knowledge of best practices in heart failure management.
  • Ensure compliance with regulatory requirements and accreditation standards.
  • Lead heart failure committee meetings.
  • Lead quality improvement projects to address gaps and serve as a clinical resources to staff.
  • Support research initiatives with heart failure team.
What qualifications you will need:
  • Bachelors Degree in Nursing required (BSN), Masters Degree in Nursing preferred.
  • Must have current RN license to practice professional nursing in the state of TX.
  • BLS certification required.
  • ACLS certification required.
  • Minimum of 3 years of clinical nursing experience.
  • Experience in quality improvement, care coordination, or accreditation processes.
  • Strong knowledge of heart failure pathophysiology, pharmacology, and evidence based guidelines.
