Position Description Essential Duties
  • Supervises, delegates, and manages all direct care given to patients by RNs, LVNs, and Nursing Aides in an appropriate manner. Ensures continuity of care by teaching and counseling the support staff.
  • Models a service and solutions‑oriented attitude towards others, takes ownership in solving problems, and takes initiative to improve services. Remains friendly and courteous to patients, visitors, and co‑workers.
  • Ensures patient, visitor, and co‑worker safety by remaining aware of and reporting any new or existing safety hazards.
  • Maintains a good attendance record.
  • Demonstrates appropriate physical assessment skills and possesses adequate knowledge of clinical diseases and disease progression. Actively participates in the coordination of all aspects of patient’s care in accordance with current professional standards.
  • Initiates communication with attending physicians, hospice staff members, and other agencies to coordinate optimal care and use of resources for the patient/family.
  • Develops and evaluates the plan of care.
  • Participates in the QAPI program and hospice‑sponsored in‑service training.
  • Participates in ongoing interdisciplinary comprehensive assessment.
  • Maintains confidentiality of all patient information, sharing only what is needed with the interdisciplinary team for proper care.
  • Maintains regular communication with the Director of Business Development, Director of Clinical Operations, and Patient Care Manager to follow up on every admission.
  • Respects patient/family rights (knocks on doors before entering, addresses by name, allows patients to make own decisions when appropriate).
  • Reviews, revises, and confirms staff assignments for the following shift, ensuring all patients are assigned to appropriate staff. Adjusts staff changes as needed and assesses patient’s clinical condition and staff productivity.
  • Completes admissions, transfers, and discharges, including assessment, documentation, hand‑off report, ordering medications/medical equipment, and other required tasks to ensure proper care and comfort.
  • Orders medications as needed and counts narcotic at shift beginning and end.
  • Performs nursing treatments and procedures as prescribed, administering them appropriately.
  • Plans the administration and/or administers medication, knowing actions, contraindications, side effects, and desired outcomes.
  • Ensures patients under direct supervision receive care and services to attain or maintain the highest practicable physical, mental, and psychosocial well‑being.
  • Provides emotional support and compassionate care, demonstrates listening skills, offers encouragement, and adjusts care to the patient’s physical condition each visit.
  • Obtains data on physical, psychological, social, and spiritual factors that may influence patient/family health status and incorporates that data into the plan of care.
  • Seeks input from other team members regarding the patient plan of care for additional knowledge and support.
  • Maintains up‑to‑date patient records so that problems, plans, actions, and goals are accurately and clearly stated and changes are reflected as they occur.
  • Instructs and educates primary caregivers, volunteers, and employed caregivers to provide indicated care; contributes to patient and family counseling and education.
  • Practices Universal Precautions according to OSHA regulations and Faith Hospice policy.
  • Notifies the MD, family, administrative staff, and direct care providers of resident change of condition and necessary follow‑up.
  • Assists other nurses, including assuming primary responsibility for patients when necessary.
  • Attends and participates in orientation, in‑services, and staff meetings to improve knowledge, skills, and maintain competency.
  • Provides support at time of death, reports death to appropriate staff, calls the medical examiner, and contacts the funeral home.
