The Atrium Health Hospice and Palliative Care team affirms the beauty of life by providing compassionate and holistic care to individuals and their loved ones who are living with a life-limiting illness.

• Monday-Friday 8:30 a.m.
  
  -5:00 pm
• Average of 3 patient visits/day;
  Minimum 15 patient visits/week
• Hybrid role of providing specialty care in home, assisted living, and nursing facilities
• Geographic Focus will be Mecklenburg County area
• Community based. Mileage reimbursement.
• Call Schedule 1:5 weeks with Hospice: M-F 5:00 pm-8:30 a.m., 24-hour coverage Saturday and Sunday
• Join a Collegial Team of 1 Physician, 2 NPs
• Academic/Teaching Opportunities

• Graduation from an accredited NP program
• 1+ years of experience in Hospice Care, Palliative Medicine, Critical Care, or Oncology is required
• Advanced Certified Hospice and Palliative Nurse (ACHPN) preferred
• Ability to work independently and autonomously
• Current prescriptive privileges and DEA license required
