Care Med Group is seeking Telehealth Nurse Practitioners to join our clinical team supporting patients through remote primary care, Chronic Care Management (CCM), and ongoing clinical care.
This is a hands‑on clinical role focused on providing high‑quality patient care, managing chronic conditions, coordinating care, and addressing changes in patient health through telehealth.
Both Full-Time and PRN opportunities are available.
Key Responsibilities Telehealth & Primary Care- Conduct telehealth visits and clinical assessments.
- Evaluate, diagnose, and manage acute and chronic conditions within scope of practice.
- Develop treatment plans and manage medications as appropriate.
- Provide patient education and preventive care.
- Coordinate care with specialists, caregivers, and other healthcare providers.
- Identify and manage patients eligible for CCM services.
- Provide ongoing care coordination for patients with multiple chronic conditions.
- Perform medication reconciliation and update individualized care plans.
- Coordinate with specialists and other members of the healthcare team.
- Conduct patient and caregiver outreach.
- Monitor patient progress and identify opportunities to improve outcomes and reduce avoidable hospitalizations.
- Maintain accurate documentation to support CCM services and billing requirements.
- Complete timely, accurate, and thorough clinical documentation.
- Maintain audit‑ready documentation supporting medical necessity and program eligibility.
- Participate in chart reviews, quality initiatives, and clinical team meetings.
- Communicate effectively with the Director of Telehealth and clinical team members.
- Active, unrestricted Nurse Practitioner/APRN license in the state(s) of practice.
- Current national board certification as a Nurse Practitioner.
- Minimum 1–2 years of clinical APRN/NP experience
. - Primary care and/or chronic disease management experience.
- Strong clinical assessment and decision‑making skills.
- Ability to work independently in a telehealth environment.
- Reliable internet connection and a private, HIPAA‑compliant workspace.
- Prior telehealth experience
. - Prior Chronic Care Management (CCM) experience
. - Experience in primary care, geriatrics, skilled nursing, assisted living, or long‑term care.
- Multi‑state APRN licensure or eligibility for additional state licensure.
- Familiarity with CCM documentation and billing requirements.
- Strong chronic disease management skills, including conditions such as diabetes, CHF, COPD, and hypertension.
- Excellent clinical assessment and patient management abilities.
- Strong communication and care coordination skills.
- Ability to work independently while collaborating with a remote clinical team.
- Strong documentation and organizational skills.
This is a fully remote, telehealth-based position
. Clinicians must maintain a private, HIPAA‑compliant workspace with reliable high‑speed internet and be available during scheduled working hours.
