This role for the Family Medicine Nurse Practitioner, provides comprehensive primary care for patients of all age groups. This role focuses on assessing patient needs, diagnosing and treating health conditions, supporting preventive care, managing chronic diseases, and empowering patients through education and individualized care planning.

Responsibilities

• Assess patients of all age groups and evaluate their health needs.
• Provide comprehensive primary care services.
• Diagnose medical conditions and develop appropriate treatment plans.
• Deliver preventive care and conduct health screenings.
• Manage chronic diseases through ongoing evaluation and care coordination.
• Manage medications and monitor treatment effectiveness.
• Educate patients on health conditions, treatment options, and preventive practices.
• Maintain accurate and timely clinical documentation.
