Key Responsibilities
  • Interview clients to determine goals or potential obstacles to reaching self-sufficiency and gather information to complete initial client assessment.
  • Create individualized plans for clients, review program requirements and set mandatory goals to be completed by the end of the program.
  • Locate resources and services to assist clients, and act as a liaison between the clients and the resource providers.
  • Monitor client progress and provide workshops and educational opportunities to assist them in reaching their goals.
  • Maintain and review client database files and ensure confidentiality of information.
  • Conduct home visits.
  • Generate monthly reports and compile statistical data.
  • Coordinate and schedule meetings and appointments.
Knowledge and Skills for Success

A successful Family Self-Sufficiency Coordinator has…

  • Knowledge of DHA organization, operations, policies, and procedures.
  • Understanding of principles and best practices in case management and counseling and applicable federal rules and regulations.
  • Expertise in conducting interviews and accessing eligibility.
  • Understanding of the principles of record keeping and records management.
  • Proficiency in utilizing a variety of business software programs.
  • The ability to effectively prioritize multiple tasks, projects and demands.
Qualifications
  • Bachelor’s degree in Behavioral Sciences, Social Work or a related field
  • Two (2) years of case management or counseling experience
  • OR an equivalent combination of education and experience.
  • Ability to obtain Family Self-Sufficiency Case Management Certificate within one (1) year of hire
