Cardiology Nurse Practitioner / Physician Assistant

Locations: Hamden, CT and Danbury, CT

Schedule: Full‑time (5 days per week)

Salary: $150,000 – $200,000

We are seeking a friendly and compassionate Cardiology Nurse Practitioner or Physician Assistant to join our team. In this role you will play a crucial part in delivering comprehensive care to patients with heart‑related conditions while working alongside a dedicated team of healthcare professionals.

• Perform comprehensive patient assessments, including physical examinations, medical histories, and diagnostic tests.
• Collaborate with cardiologists to develop and implement treatment plans for cardiovascular patients.
• Administer medications, monitor patient responses, and adjust treatment plans as necessary.
• Conduct cardiac stress tests, electrocardiograms (EKGs), and other diagnostic procedures.
• Educate patients and families about cardiovascular health, preventive measures, and medication adherence.
• Provide counseling and emotional support to patients and families.
• Manage medication, wound care, and fall prevention strategies for residents.
• Maintain knowledge of CMS regulations and NCQA HEDIS guidelines.
• Document accurate medical records to ensure compliance with OSHA and other regulatory standards.
• Collaborate with the interdisciplinary team and analyze care of critically complex patients.

• Minimum 6 months of experience in long‑term care or a skilled nursing facility.
• Active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license.
• DEA license.
• CPR BCLS/BLS certification.
• Master’s Degree in Nursing.
• Completed a degree from an accredited Nurse Practitioner program.
• Preferred: experience in cardiology (training provided).
• Preferred: experience documenting EMR (Sigma, Visual, PCC, etc.) (training provided).
