Hartford Health Care at Home is seeking an experienced RN for weekend OASIS visits in CT. You will perform comprehensive nursing assessments, complete OASIS Time Point data, develop plans of care with interdisciplinary teams, and coordinate referrals and caregiver education.
The role requires CT RN licensure and prior home health experience with Oasis knowledge. You will lead a care team, supervise LPNs and aides, and ensure accurate documentation and SBAR reporting in a fast-paced home setting.
