Summary:
The Aortic Center Nurse Navigator is a specialized Registered Nurse responsible for coordinating the complex care of patients with aortic diseases. The Nurse Navigator coordinates multidisciplinary teams, facilitates timely appointments, educates patients and families, and removes barriers to care, acting as the primary point of patient contact to ensure a seamless continuum of care for the patient. This role reports directly to the Aortic Center Program Director.

- Acts as a liaison for clinical questions, triaging patient calls and adjusting medications under physician direction before escalating to advanced practitioners. (15%)
- Collaborates with the multidisciplinary team to maintain patient registries and track clinical outcomes for program growth.(10%)
- Graduate of an accredited school of nursing.
- Bachelor of Science in Nursing (BSN) preferred.
- Current Registered Nurse (RN) license in the state of practice.
- Current BLS certification required.
- ACL S certification preferred.
- Minimum of 3 years of clinical nursing experience.
- Experience in vascular surgery desired.
- Prior patient navigation, care coordination, or case management experience preferred.
- Experience with electronic medical records (EMR) systems.
