Hartford Health Care’s Neighborhood Health program is seeking passionate per diem Advanced Practitioners to join our primary care team dedicated to improving health equity in underserved communities . This role offers the opportunity to provide high-quality preventative and urgent care through mobile and community-based services, addressing health disparities and promoting wellness. Greater Norwich, CT area

Setting : 100% Outpatient Primary Care/Urgent Care – Family Medicine

Schedule :
Full-time, weekdays only.

Nurse Practitioners and Physician Assistants Role Scope:

Under established guidelines and in collaboration and/or supervision with the practice physician(s), Nurse Practitioners and Physician Assistants perform routine physical examinations, and formulate medical, educational, and psychosocial plans to treat patients.

In consultation with the physician(s) and care team members, Nurse Practitioners and Physician Assistants develop plans for further evaluation of health needs, e.g., referral to other health team members or specialists.

Nurse Practitioners or Physician Assistants provide appropriate treatment for problems within the scope of the provider’s practice as designated by protocols and identifies problems outside of their scope of practice and refers appropriately to physician.

Qualifications

Requirements:

Licensure :
Unrestricted State of Connecticut Medical License (or eligible)

·
Training :
Family Nurse Practitioner (FNP) or Physician Assistant (PA)

·
Experience : 2+ years as a Family Nurse Practitioner or Physician Assistant, preferably in a primary care or urgent care setting.
