RN Case Manager – Full-Time, Flexible schedule (40 Hours/Week)

As a vital member of our interdisciplinary team, you’ll work under the supervision of a Clinical Manager to coordinate all aspects of patient care—ensuring individualized, compassionate support that maximizes quality of life.

• Conduct ongoing, holistic assessments of patients and families
• Develop and manage comprehensive care plans in coordination with the care team
• Facilitate interdisciplinary case conferences and care planning discussions
• Collaborate with physicians, hospice staff, volunteers, and external agencies to ensure optimal support
• Advocate for patients and families, addressing physical, emotional, and psychosocial needs

• Active Massachusetts RN license in good standing
• Associate's degree in Nursing required; BSN or equivalent preferred
• Proven ability to provide care with compassion and cultural sensitivity in diverse home or facility settings
• Strong interpersonal skills and the ability to thrive in a collaborative team environment
• Flexibility, self-direction, and sound clinical judgment
• Valid driver's license and access to an insured, reliable vehicle

The pay range listed is the anticipated hiring range. Precise pay rates are determined by factors such as education, training, relevant experience and work location.

Pay Range: $37—$52 USD

Benefits are offered to employees scheduled to work 20+ hours/week, including earned time, tuition reimbursement, student loan paydown, retirement plans, and medical/dental/vision/life/disability insurance.
