The LPN is responsible to provide direct care to individuals living in a residential setting. He or she will follow the outlined plan of care, identify any health-related needs, monitor medical status and report changes to the on-call Registered Nurse, and work with residential staff to support the individuals in their community residence.

• Understand the clinical and social needs of the individuals’ receiving services.

Utilize nursing knowledge and skills in the safe implementation of basic preventative therapeutic and rehabilitative nursing care.
• Assess individual throughout the shift to determine nursing needs. Report to and seek direction from the RN Case Manager or on-call RN as needed.
• Present information regarding any individual in a professional manner in all settings including meetings, phone contact, daily written documentation and /or email correspondences.
• Responsible to assist at physician’s appointments, community trips, and other activities as assigned.
• Prepare and teach health related in service to staff and provide ongoing supervision of ADL and other health related routines of each individual.

• Licensed LPN in MASS – New Grads Welcome
• Experience with intellectual/developmentally disabled individuals a plus
• Must have a valid Driver’s license and proof of a good driving record.
