• Coordinate and deliver high-quality hospice care in accordance with physician orders and organizational policies.
• Conduct comprehensive assessments of patient health, psychosocial, and spiritual needs.
• Develop and update individualized care plans, ensuring ongoing evaluation and adjustment.
• Provide direct patient care and supervise ancillary staff, including hospice aides and volunteers.
• Facilitate communication with physicians, families, and other healthcare providers to ensure seamless care.
• Participate in interdisciplinary team meetings, case conferences, and quality improvement initiatives.
• Support families through bereavement and provide follow-up care as needed.
• Maintain accurate documentation of all patient interactions and care activities.
• Assist with discharge planning and coordinate resources for continued support.

• Valid RN license in Massachusetts in good standing.
• Associate’s degree in nursing required;
  Bachelor’s degree preferred.
• Minimum of three years of nursing experience, including at least one year in an acute care setting.
• Strong assessment skills and ability to respond effectively to diverse patient needs.
• Knowledge of hospice philosophy, principles, and practices.
• Excellent communication and teamwork skills.
• Valid driver’s license, insured vehicle, and willingness to travel within the Danvers, MA area.
• Ability to work independently, demonstrate flexibility, and handle emotional aspects of hospice care.

• Competitive hourly pay between $45 and $60 based on experience.
