We are looking for a full-time Program Case Manager to join our Day Services team!

Schedule: Monday-Friday 8 am-4 pm

Pay Rate: $18.50/hour

Job Summary: Provides supervision, opportunities, and skill development to persons served by the Northeast Arc in their community-based day and work support program.

Major Responsibilities:

• Evaluates and determines program participants' interests and develops individual skills.
• Provides oversight to assigned caseload by attending scheduled meetings, maintaining regular and professional contact with the participant's team, completing all required documentation, and ensuring the participant's progress in the program as determined by his or her goals.
• Creates individual goals to reflect the participant's vision regarding skill development.
• Maintains all required documentation and paperwork within required time frames.
• Makes sound clinical decisions and appropriate interventions but seeks assistance from a supervisor when necessary.
• Provides transportation to persons served in a personal vehicle or agency vehicle when one is available.
• Assist in obtaining appropriate volunteer placements for persons served and provide ongoing coaching support in these placements to ensure continued success.
• Provides oversight for a group of people working in a company (if applicable).
• Ensures all work completed within group employment sites meets the expectations of the company (if applicable).
• Attends and completes all required training assigned by the supervisor.
• Follows HIPPA, security regulations, and/or guidelines and always maintains confidentiality.
• Other duties and special projects as assigned.

Qualifications:

• B.A., B.S. in psychology or related field preferred
• Prior experience working with intellectually and/or developmentally disabled adults or other related experiences.
• Excellent organizational and interpersonal skills. Effective communicator in both person and writing.
• A valid Mass driver's license and reliable transportation are required.
• Must be able to lift up to 50 lbs as needed

Work Environment:
The work environment changes daily. Program Case Managers work in office space, program areas, at volunteer sites, in vehicles, and in various locations out in the community during outings.
