Registered Nurse - Advanced Acute Care Transitions

Inpatient Registered Nurse providing compassionate and comprehensive patient care in a specialized unit.

Conduct assessments, develop and coordinate individualized care plans, administer treatments and medications, and evaluate patient responses. Collaborate with interdisciplinary teams and physicians to ensure optimal patient outcomes. Promote safety, patient satisfaction, and emotional/spiritual support. Participate in quality improvement and safety initiatives, troubleshoot equipment issues, and contribute to evidence-based practice discussions.

Nursing degree from an accredited program, with the ability to provide holistic care addressing physical, psychological, social, cultural, and spiritual needs. Strong communication, teamwork, problem-solving, and critical thinking skills. Ability to adapt in a calm, supportive environment.

Assess patient needs, formulate care plans, deliver direct care, monitor treatment effectiveness, and collaborate with healthcare team members. Engage actively in safety and quality programs, and uphold organizational policies and standards.

Focus on palliative, hospice, and medical-surgical care with a focus on emotional and spiritual support.
