Registered Nurse Navigator - Pediatrics Allergy

Provides patient education, promotes understanding of health conditions, and ensures continuity of care. Acts as a patient navigator, coordinating pre- and post-hospital care, and facilitating entrance into care programs. Serves as a liaison between patients and healthcare teams, supporting multidisciplinary clinics and activities.

• Coordinate patient activities across interdisciplinary teams and clinics.
• Educate patients and families about conditions and care plans, both in person and via phone/mail.
• Support diagnosis and treatment by providing clinical assistance to providers and patients.
• Communicate effectively with patients, ensuring timely follow-up and surveillance.
• Coordinate support services such as support groups and social services.
• Maintain systems for patient follow-up and clinical documentation.
• Provide nursing care and assist with procedures as needed.
• Educate team members and act as a resource within the clinical environment.

• Graduate from a specialty nursing training program (required);
  Bachelor's Degree in Nursing preferred.
• Minimum of two years RN experience.
• Strong communication, organizational, and patient advocacy skills.
• Ability to collaborate effectively with multidisciplinary teams.
