Registered Nurse Navigator - Pediatrics Allergy

Provides patient education, coordinates care, and serves as a liaison between patients and healthcare teams to ensure continuity and quality of care. Facilitates patient entry into care programs, supports pre- and post-hospital care, and collaborates with multidisciplinary teams to develop and implement treatment plans.

• Guide new patients into care programs and coordinate appropriate clinic services.
• Educate patients about their conditions and promote understanding for ongoing care.
• Assist in diagnosis, treatment, follow-up, and surveillance, acting as a clinical support resource.
• Communicate with patients via mail, phone, and in person, and coordinate activities with support services like social support and support groups.
• Maintain systems for timely follow-up and ensure compliance with policies.
• Support physicians and assist with procedures, while educating team members in relevant areas.

• Graduate of Specialty Training Program in Nursing (required);
  Bachelor's Degree in Nursing (preferred).
• At least two years of RN experience.
• Strong communication, coordination, and patient education skills.
• Ability to collaborate effectively with multidisciplinary teams in a clinical setting.
