Registered Nurse - Acute Care Transitions

This is a full-time Inpatient RN position focusing on Palliative, Hospice, and Medical Surgical care. The unit features 14 private, renovated beds within a calm and supportive environment, emphasizing emotionally and spiritually supportive care.

• Assess, plan, perform, and evaluate nursing activities to meet patient needs and promote safety and satisfaction.
• Provide compassionate, patient-centered care, including treatments, medications, and diagnostic studies.
• Coordinate interdisciplinary care, including patient and family education, discharge planning, and response to patient responses.
• Collaborate proactively with physicians and team members to address patient needs and resolve issues.
• Participate in quality and safety initiatives, unit-based decision-making, and evidence-based practice.
• Troubleshoot equipment, problem solve, and ensure compliance with policies and standards.

• Graduate from a Specialty Training Program in Nursing (required).
• Strong assessment, communication, and teamwork skills.
