Hospice RN Case Manager

As a Hospice RN Case Manager, you will take full nursing responsibility for the delivery of the Plan of Care for each hospice patient, collaborating with other professionals to develop and implement treatment strategies that promote overall well-being.

• Pay: $75,624 - $100,352/year

• Make the initial nursing evaluation to determine eligibility for hospice services within 48 hours of referral.
• Identify the patient's/family's physical, psychosocial, and environmental needs, reassessing no less than every 15 days.
• Initiate and coordinate the plan of care.
• Document problems, appropriate goals, interventions, and patient/family responses to hospice care.
• Collaborate with the patient/family, attending physician, and other members of the IDG in providing daily care.
• Instruct and supervise the patient/family in self-care techniques when appropriate.
• Maintain accurate and relevant clinical notes regarding the patient's condition.
• Function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Education:

• Current and unrestricted RN licensure in the state of practice.

Experience:

• 1+ years of clinical experience preferred.

• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
