Registered Nurse - Case Manager

RN Case Manager supporting patient-centered care coordination within a healthcare plan.

• Assess, plan, implement, and evaluate care options to optimize health outcomes.
• Develop individualized care plans integrating evidence-based guidelines and promoting quality and efficiency.
• Monitor progress, manage utilization, and promote early intervention in acute situations.
• Collaborate with patients, families, providers, and healthcare teams to ensure coordinated services.
• Analyze clinical data, laboratory results, and utilization patterns to improve care delivery.
• Maintain documentation and adjust treatment plans based on data insights.
• Support advanced patient-centered care models and promote continuous quality improvement.

• Minimum two years RN work experience; case management and CCM/ACMA certification preferred.
• BSN preferred; knowledge of healthcare laws, regulations, insurance policies, and healthcare IT systems advantageous.
• Ability to work independently and collaboratively; active RN license is a plus.
