Acts as a member of a training and care team and fosters continuity of care across all shifts and promotes smooth and efficient program and facility operation. Assists with the assessment, development, and implementation of individual service plans. Provides quality care, training, support and supervision. Monitors and maintains the safety and wellness of individuals. Assists individuals with self‑help, task learning, and community utilization skills based on individual goals;

maintains documentation according to all applicable standards. May function without direct oversight in accordance with standard operating procedures.

• M1: High school diploma or GED
• M2: Valid driver’s license
• M3: Must be physically able to push wheelchairs, assist with transfers and be on your feet for extended period of time

Fingerprints, State and FBI criminal record reports, drug test, and central registry (CANIS) report will be required upon request. Our Agency maintains a drug‑free workplace.

Hourly Rate Range: $15.62 - $27.33

Shift Differential: $2.00 2nd Shift, $3.00 3rd Shift
3% Increase for Certified Nursing Assistant
