Commonwealth Hospice, a part of LHC Group and a member of the Optum family, seeks a Hospice Nurse Practitioner to provide consultation in palliative care, symptom management, and supportive care for hospice patients.
This PRN/as-needed position offers face-to-face visits with weekday hours and potential evenings/weekends. The NP will coordinate care plans with the Hospice Medical Director and interdisciplinary team, documenting patient progress and educating families as needed.
