Hospice Triage Nurse (Remote)

• Answer all incoming calls and communications and provide telehealth assessments, pain management, and symptom control.
• Assess the patient and family's need for a home visit and schedule through HCHB, notifying the patient and family of the expected visit time.
• Manage HCHB workflow, including documentation of coordination notes and back‑office workflow.
• Serve as an after‑hours point of contact to ensure referrals are documented and admissions are timely.
• Coordinate patient care and communicate with interdisciplinary teams.

• Practical Nurse, Vocational Nurse, or Registered Nurse licensed in the state of employment.
• Minimum 3 years of hospice nursing experience.
• Remote access Wi‑Fi and phone.
• Demonstrated expertise in pain management, symptom control, crisis intervention, and emotional and pre‑bereavement support.
• Excellent critical thinking skills and clinical judgment.
• Previous experience operating and documenting in Homecare Home‑Based.
• Excellent verbal and written communication skills.
• Self‑directed and able to work with minimal direct supervision.
• Ability to work outside of posted regular business hours, including overnights and weekends.
