• Maintains a professional image and exhibits excellent customer relations to patients, visitors, physicians, and co-workers in accordance with Service Excellence Standards and Core Values.
• Assists with revisits within a geographical area, supervised by the Team Clinical Manager and working under the primary RN, collaborating with all caregiver staff (team RN, PT, OT, ST, MSW) for interdisciplinary coordination of patient care and performing independent actions consistent with professional standards of nursing practice.
• Demonstrates good communication, attention to guest relations, interpersonal relationships, writing skills, and computer literacy; organizational and time‑management skills desirable.
• Handles potential problems related to safety issues for staff and patients, is familiar with growth and development principles across the life span, and assesses data reflective of the patient’s status and interpretation of information needed for each patient’s age‑specific needs; requires maturity, self‑motivation, creativity, initiative, independent judgment, responsible, ethical, self‑initiating professional behavior.
• Utilizes effective interpersonal skills, empathy, active listening; plans with staff, patients, customers, and physicians; communicates efficiently with a diverse population; self‑awareness and self‑evaluation; accurate data collection, record completion, follow‑up; works closely with staff, physicians, external agencies; adheres to dress code, presents a neat, clean, well‑groomed appearance, follows attendance policy, and demonstrates punctuality and good attendance.

Full Time, Day Shift. This position covers Darlington and Lee counties.

• At least one year of nursing experience required.
• One year of home care experience highly desirable.
• Previous home care experience preferred.

• Current valid SC driver’s license and automobile insurance as required by law; must be at least 21 years old for fleet vehicle insurance policy coverage and meet driving eligibility as per McLeod policy; license and registration as required by state law.
• LPN license, BLS certification (American Heart Association).
• Possession of professional liability insurance encouraged.
