Band 5 Community Staff Nurse

Overview

Band 5 Community Staff Nurse, joining Swanley and Rural PCN Community Nursing Team in Dartford, North Kent. Deliver care in patients' homes and local communities to help patients remain independent and avoid unnecessary hospital admissions. Autonomous role with strong relationships with patients and carers and opportunities to develop clinical skills in a community setting. Newly qualified or experienced nurses are welcome.

• Deliver safe, effective and compassionate nursing care in the community and in patients' homes.
• Assess patient needs and contribute to care planning.
• Monitor patient progress and escalate concerns where appropriate.
• Support patients and carers with information and advice.
• Work collaboratively with the multidisciplinary team and external partners.
• Maintain accurate and up-to-date clinical records.
• Contribute to service improvement and quality care delivery.

Essential

• Registered Nurse with a valid NMC pin (Adult).
• Strong communication and interpersonal skills.
• Ability to work both independently and as part of a team.
• Good organisational skills and ability to manage your workload.
• Caring and patient-focused approach.
• Confidence using IT systems, including Microsoft Office.

Desirable

• Experience with in a community or primary care setting.
• Experience working with patients with long-term conditions.
• Understanding of admission avoidance and community-based care.

Other requirements

• Full UK driving license and access to a vehicle for work purposes.
