Lead Special Patient Program in Healthcare

Drive patient-centered care as the Special Patient Program Lead at EMC in Dartmouth. Oversee program approvals, collaborate with teams, and develop comprehensive care plans in this impactful role.

Reporting to the Manager of Interprofessional Practice & Learning, the Special Patient Program Lead will manage the EHS SPP with a focus on effective implementation and ongoing enhancement. Your expertise in healthcare, along with strong organizational skills, will be essential in processing applications, collaborating with various healthcare partners, and ensuring patient needs are met satisfactorily.

Key Responsibilities:

• Process, amend, and approve SPP applications efficiently

• Develop and revise clinical and operational care plans

• Collaborate with healthcare providers and EHS teams closely

• Communicate effectively with internal and external partners

• Maintain accurate records and support program administration

Requirements:

• Regulated Healthcare Professional in Nova Scotia

• Experience in a healthcare setting handling sensitive information

• Strong analytical and problem-solving skills

• Exceptional communication and organizational abilities

• Proficient in Microsoft Office applications
